CloudForms SmartProxy Server Role should be selected by default.

Steps to Reproduce:
1. Install RHCI/TripleO
2. Deploy OpenStack + CloudForms
3. CloudForms: Configure > Configuration
4. Settings accordion
5. Server "EVM [1000000000001]"
6. Server tab
7. Server Control > Server Roles

Actual results:

SmartProxy not selected

Expected results:

SmartProxy selected
