Backport bug fixes from upstream.

It appears this isn't as simple as just grabbing any stable 4.0.y patches, we also need to look at 3.10.y (and other branches) since not everything made it into stable-4.0.y, unfortunately.

specifically,

iscsi-target: Fix use-after-free during TPG session shutdown

was missed and encountered by a user.
