How reproducible:
100%

Steps to Reproduce:
1.Navigate to Compute->Infrastructure->Virtual Machines  
2.choose VM and Click on "Migrate selected item" under Lifecycle 

Actual results:
For vm migration Submit and Cancel buttons are not visible. 

Expected results:
For vm migration Submit and Cancel buttons should be visible.

For vm migration Submit and Cancel buttons are visible and able to migrate Vm.

Verified in 5.6.0.6-beta2.5.20160511140943_ff75fb2
