Description of problem:
Can't add new cluster to the latest 4.0 ovirt. 

A Uncaught exception occurred. Please try reloading the page. Details: Exception caught: (TypeError) __gwt$exception: <skipped>: Cannot read property 'k' of undefined
Please have your administrator check the UI logs
