Description of problem:

The current provisioner pod does not contain Namespace in logging statements for PVCs.  Improving all logging statements will help Ops manage this pod.

Version-Release number of selected component (if applicable):

Logging is improved in 3.2.0.1 version of the volume-provisioner image.

Closing, as this BZ is no longer needed for QE to test logging improvements for prod.  The cause of our error was found, thus negating the need for enhanced logging.

These improvements will be rolled into a regular PR and feature update.
