Description of problem:
'Image Upload' dialog - as uploading using a URI isn't currently supported, 'Image Source' panel should be removed.

Steps to Reproduce:
1. Open 'Image Upload' dialog

Actual results:
There's an 'Image Source' panel for selecting between 'Local File' and 'Remote URI'.

Expected results:
No visible 'Image Source' panel.

Remote URI is not working? is there a bug for it for 4.1 to get the functionality back?

AFAIK, it was never implemented.. Should open an RFE for that.

It's part of 4.1 content, shouldn't be visible in 4.0.
