Steps to Reproduce:
1. Create a service request in CFME . 
2. In SUI , Request page - two requests show up after polling .
3. The additional request shows "provisioning from template to vm_name" as shown in screenshot .

I took a look and this is most likely some sort of error upstream with the API.  Let me know if you would like me to look at this in the future if it continues to happen and the API team has had a look.
