Description of problem: Error indicator does not display on the OpenStack New Infrastructure Provider form for the Default tab

Steps to Reproduce:
1. Compute -> Infrastructure -> Providers
2. Configuration -> Add New Infra Provider
3. Select OpenStack from Type dropdown

Actual results:
No Error indicator shown on Default Tab

Expected results:
Error indicator should be visible on Default Tab
