Prior to OSP12, lvm modules were not included in the ramdisk, so whole disk images with multiple images were unable to mount.

Document in the section that describes whole disk images.  Also include a note in previous version's documentation describing this limitation.

Hi , this is just part of the needed code. But we also needed https://review.openstack.org/#/c/499588/5/elements/overcloud-secure/block-device-default.yaml , that is targetted for Queens
