Description of problem:
It seems the Orders in the SSUI are always seen as:
Approved, Request State: Finished.
and not reflecting Service requests failures.

For example, 
in case of Ordering a service VM provision, on RHV provider that was added with ca_certificate.
On CFME UI, Request fail (bug 1483303), however on Self service UI, the failure is not reflected.

Can you provide screenshots of what shows up in CFME UI and also what you see in the SUI.

As can be seen in the 2 screenshots attached in Adminisration UI some of the service requests failed, but in the self service UI, those same service requests status failed is not reflected, and seem as if those service requests finished OK.

Verified on:
CFME 5.9.0.14
RHV 4.1.7.6

Verification steps:
1. Create a catalog item for VM provision that will be successful and order it from CFME. Wait for it to complete.
2. Go to SSUI -> My Orders.
3. That order should have Request State: Finished.
4. Create a catalog item for VM provision that will fail. (One possible way is just take the previous catalog item and in RHV remove that template that is used by the catalog item.) Order it and wait for it to fail.
5. Go to SSUI -> My Orders.
6. That order should have Request State: Error.
