Description of problem:
Network field is missing while creating

Version-Release number of selected component (if applicable):
5.8.3

How reproducible:
100%

Steps to Reproduce:
1. Add OpenStack provider.
2. Navigate to Network -> Subnets
3. Open add new subnet page
4. Check page UI

Actual results:
Page is corrupted, network dropdown is missing

Expected results:
Network drop-down should be present on the page
