Description of problem:

1) Update install documentation by removing references to enabling the rhscon repositories when installing ceph-ansible from Installation Guide for RHEL and Installation Guide for Ubuntu.

2)  Remove salt from ceph-repos (Ref: https://bugzilla.redhat.com/show_bug.cgi?id=1507645)
