Description of problem:
When provisioning VM in Azure through Lifecycle, errors do not appear in UI for not filling in Username or Password under "Customize" tab.

Version-Release number of selected component (if applicable):
5.8.x

How reproducible:
Every time

Steps to Reproduce:
1.  Go Compute > Cloud > Instances > Lifecycle > Provision Instance
2. a. Fill out everything but Username and/or Password and click submit
   b.  Fill out a password or username that doesn't meet Azure's requirements
3.  No errors appear in UI

Actual results:
No error, no loading screen, just sits there.

Expected results:
The errors should appear in the UI for either not filling in Username, password, or filling in these fields that don't meet Azure's requirements

Additional info:
Cannot reproduce in 5.9.0.17, but can reproduce locally on own 5.8.2.3 instance.
