Dave, it's working on 5.8.3.4, >=5.9.0.11. In 5.9.0.12 was introduced different bug, which didn't display all the fields on that page. In 5.9.0.15 it was fixed but with improper formatting. I think in the time I was testing this there was 5.9.0.21 just introduced so I was still testing in 5.9.0.20 and reported it at time there was 5.9.0.21.
Verified in 5.10.0.3. Page is styled properly.
