Description of problem:
Horizon launch instance panel show is showing inconsistent "Select Boot Source". On the first attempt, it defaults to "Image", however on the second and further, it changes between "Volume Snapshot" or "Image".

This confuses users and should only have one option by default.

A bug has filed https://bugs.launchpad.net/horizon/+bug/1640493 and requesting the patch to be back-ported to  RHOSP 10 (Newton)
