Infra Mapping list item with errors

Description of problem:

There are at least two scenarios that can cause the Infra Mapping list items to look like the attached screenshot

1. A provider was removed via Compute -> Infrastructure -> Providers
2. A cluster was removed via Compute -> Infrastructure -> Clusters

We would like to provide more detailed information regarding why the clusters/networks/datastores were unable to be retrieved.

NOTE: When a cluster is removed, so are all of its associated networks and datastores. So in either of the scenarios listed above, the list item will look the same

This PR provides a simple error message related to this BZ, and should be backported to hammer: https://github.com/ManageIQ/manageiq-v2v/pull/906
We should leave the BZ open however, more robust error messages are in the works for a later release.

Fabien, maybe we need to move this one to POST so the PR can be backported, then open another BZ to represent the future work for enhanced mapping validation?
