Description of problem:

While trying to create an objectclass facing rendering issue in Add Objectclass
dialogue window.

Version-Release number of selected component (if applicable):

389-ds-base-1.4.1.1-20190312git813030cc7.el8.x86_64

How reproducible:

Every time

Steps to Reproduce:

1. Create an LDAP instance, click on schema tab, under that click on create ObjectClass


Actual results:

Rendering issue.

Expected results:

Should open proper dialogue box.

Still happens, both in Firefox and Chrome.
