When an upgrade is failing, the CVO reports two series for cluster_version which complicates all of our count queries where we want to find clusters that have versions.  Now that we have from_version, double reporting isn't useful.  We should report only a single series and set from_version appropriately.

Tested with upgrade, CVO reports one failure series for cluster_version now
