Description of problem:

Whatever attribute you choose to add as an index, the UI sets the name to "undefined"

dsconf -j ldapi://%2fvar%2frun%2fslapd-localhost.socket backend index add --attr=undefined o=test --index-type=eq

Build tested: cockpit-389-ds-1.4.2.12-1.module+el8dsrv+6328+f04d7471.noarch

While creating an index, I see in the console:
CMD: saveIndex: Create new index ==> dsconf -j ldapi://%2fvar%2frun%2fslapd-test.socket backend index add --attr=initials dc=example,dc=com --index-type=eq --index-type=pres --index-type=sub --reindex

Name is correct, marking as VERIFIED.
