Description of problem:
In the mig plan wizard, it's extremely jarring to have the form dynamically show or hide text fields depending on whether or not the user selects source or target radio buttons. It causes entire sections of the form to potentially get rendered out of view (like the target section) where the user would potentially not realize they need to scroll the modal (which is not a great experience regardless)

Note: May require certain levels of zoomed in browser to demonstrate the pain

How reproducible:
Every time

Steps to Reproduce:
1. Navigate to hooks screen and try adding a hook, flip between source and target radio buttons, they jump around


Expected results:
The form should not be bouncing around the page

This has been fixed actually in a previous release. I'm moving to MODIFIED so Rayford can attach to the errata and we can get this verified.

verified with MTC 1.4.3.
