Description of problem: Replace "Are you sure" text when confirming deletion of providers, mappings and plans

How reproducible: Every time


Steps to Reproduce:
1. Attempt to remove a provider, migration plan, mapping, or hook
2. Or attempt to leave while in the middle of creating a plan
3. Notice the language prompted to the user

Actual results: Wording uses "Are you sure"


Expected results: Make title a question, and explain what will happen if user proceeds.

We received some additional feedback after merging and backporting, so we'd like to keep this BZ open until https://github.com/konveyor/forklift-ui/issues/706 is resolved. Sorry for the churn.
