Description of problem:
App Name and Name field values are not getting auto-populated after filling Internal Registry details on Deploy Image Page

How reproducible:
Always

Steps to Reproduce:
1. In Developer Perspective, Click Add on Left side navigation menu
2. Click  "Container Images" card on Add page
3. Select "Image stream tag from internal registry" Radio button
4. Select "Openshift" project
5. Select "golang" Image stream
6. Select "latest" tag

Actual results:
Application Name and Name filed values are not auto-populated

Expected results:
Application Name and Name filed values should be auto-populated
