Description of problem:
Click "This is a CD-ROM boot source" ticking "Use template size PVC" on pvc upload form.
It seems "This is a CD-ROM boot source" is not labelling correctly.

Version-Release number of selected component (if applicable):
4.9.0-fc.0

How reproducible:
100%

Steps to Reproduce:
1. Go to Storage -> PVC -> With data upload form
2. Click text "This is a CD-ROM boot source"
3. The above item is checking and unchecking by clicking this one.

Indeed, the PVC upload form is changed on master, "Attach this data to a Virtual Machine operating system" is removed on the page.

Yes, with the move to auto-upload images and using sourceRef, we removed the option to attach an uploaded image to a specific operating system using a form.

Uploading to a specific operating system will be manged by the back end, we will add UI for the auto-upload mechanism in the future,
currently we just removed the detracted option to manually upload to specific OS, a user can still upload images without specifying the target OS.

Moving this bug to WANTFIX because the feature was removed from the UI.

Reopening until we will stop supporting non DataSource templates.

Clicking the labels ticks proper checkboxes.
