Description of problem:
Changing configuration in Account Policy Plugin produces error message.

Steps to Reproduce:
1. Go to Plugins tab -> Click Account Policy
2. Enable plugin -> restart
3. Click Create Config
4. Check "Always Record Login"
5. Click Add config


Actual results:
A message is displayed: Error during the config entry add operation - list index out of range

There is no error when editing config correctly, marking as VERIFIED.
