Host > Add form misaligned

Description of problem:

The Hosts > Add form is misaligned for the dashboard.
It seems to be cut off at the top.

How reproducible:

1/1

Steps to Reproduce:
1. Deploy an RHCS 8.0 cluster
2. Access dashboard Hosts > Add form
3.

Actual results:

Form is misaligned

Expected results:

Form must be correctly aligned
