Add the callbacks for rport device loss timeout, and set the timeouts
consistently. Also remove the drivers routine get_rport_dev_loss_tmo().

These are backports from the following upstream commits. They have been
tested at QLogic.


    qla2xxx: Set an rport's dev_loss_tmo value in a consistent manner.

This request was evaluated by Red Hat Product Management for inclusion in a Red
Hat Enterprise Linux maintenance release.  Product Management has requested
further review of this request by Red Hat Engineering, for potential
inclusion in a Red Hat Enterprise Linux Update release for currently deployed
products.  This request is not yet committed for inclusion in an Update
release.
