Fixes from testing with BladeEngine3 device

This patch has three changes required for the driver to work with BladeEngine3 device. Two of the fixes are alredy applied upstream. (Commit ids
7b139c83c590d4965259aad8889cbb08104b2891 and d291b9af1a1a12f59a464494900c6e0db26e2ec3).

The third one for change in the firmware image layout will be  submitted shortly.

further review of this request by Red Hat Engineering, for potential
inclusion in a Red Hat Enterprise Linux Update release for currently deployed
products.  This request is not yet committed for inclusion in an Update
release.

After speaking to Laurie - she didn't think a firmware update was required, only the two patches.

Andrius - there's been some confusion.  This last change is required to the driver for BE3 fw download support.  I misunderstood you on today's call saying that the actual firmware itself was being submitted upstream and into the distros which is not required or desirable to Emulex.

Andrius, yes, two are upstream now but the following were submitted to netdev@... but were not applied by Dave yet:

be2net: Bug fix to support newer generation of BE ASIC
be2net: bug fix in be_change_mtu
be2net: bug fix for flashing the BladeEngine3 ASIC
be2net: remove ASIC generation number from Kconfig
be2net: change the driver description
be2net: bug fix in be_read_eeprom
