The reproducer remains unknown.
According http://lkml.org/lkml/2008/3/12/207 the bug was reproduced on eight Xeon machine. I've tested heavy aio multithreaded reads with aio-stress on machine with eight Xeons. I've never triggered the hang on unpatched -128.25.1.el5 nor patched -128.26.1.el5.
