Further digging reveals that this is most likely a uCode firmware bug that seems to be affecting a series of Intel cards so I guess 4965 can be added to that list. See https://bugzilla.kernel.org/show_bug.cgi?id=16691 for details This is the firmware I got installed. /lib/firmware/iwlwifi-4965-1.ucode /lib/firmware/iwlwifi-4965-2.ucode iwlwifi-4965-2.ucode is what's being shipped in iwlwifi-4965-ucode-228.61.2.24 which is the latest available for this card on http://intellinuxwireless.org/?n=Downloads
