It is not entirely clear that once you get to the second screen shot that you should click on the Insert [customer] part of the screen to get the pop up box to add in more fields.  The Add Field button should always show up.  I struggled for a while with this, and would love to have this fixed from a usability perspective.

This is the way it works before any fields are added, this needs to always show up.

The second screen shot shows how it disappears which I do not think is the right paradigm.
