Description of problem:
When building a translated book, the merged Revision_History.xml file is missing its DOCTYPE declaration. If the file contains DocBook entities (for example,  ) the book will not build

publican-3.0-0.fc17.t219.noarch


How reproducible:
100%

Steps to Reproduce:
1. run publican build for a translated book
2. inspect the head of tmp/<LANG>/xml_tmp/Revision_History.xml
  
Actual results:
No DOCTYPE declaration

Expected results:
DOCTYPE declaration

Additional info:
1. Add &nbsp; somewhere in a <member> in the en-US/Revision_History.xml file
2. Run publican build

Build fails with something like:

undefined entity at line 97, column 20, byte 2613:
