3. Created Application BP from the image and launched instance
4. Instance shows Running state with IP displayed . (attachment 1 [details])
5. Monitor Page dashboard shows Red.
6. Filter View shows instance state as Running.
7. Filter View Application says "Failed Application"

I am also hitting the same isssue.
sometimes the instance automatically goes in vanished state and sometimes running.

I am thinking this is related to issues with the vsphere environment itself.  Closing this until we have a chance to address the issues and retest,
