think this was caused by doing a pull from within eclipse

I assume you were using tool which sent incorrect data to Mercurial? Which tool was that? A manually install Mercurial-eclipse?

yes i'm using eclipse 4.2.1 and MercurialEclipse 2.0.0.v201210091230

So the real problem is apparently that MercurialEclipse is sending invalid data to Mercurial. You should file a bug there, especially if you can reproduce the problem.
