What was going on with the machine when this happened?  Is it easily recreated?

Not very easily. It recreates some time if I attach to laptop external USB3.0 hard drive and active use it. I dont see depends to started processes on host, because it can reproduce if I start only vlc player, or if I start eclipse/firefox/thunderbird/...

Are you still seeing this with 3.9.8 or newer?  If so, please attach the full dmesg output.
