Description of problem:
I'm trying to add a button for providers on cloudforms 4.1beta 2.3 but I can't. When I go to Automate/Customization/Buttons/ and select any object type and try to add a button group or a button the new frame opens with the label "Adding a new Buttons Group" but there is no Basic info and input boxes to enter text or images
I've tried with user admin group EvmGroup-super_administrator and with a fresh user also member of the EvmGroup-super_administrator group.

How reproducible:

100% Reproducible following the steps below.

Steps to Reproduce:
1. Navigate to Automate/Customization/Button
2. Add a button group or add a button 

Actual results:
No input fields or labels.
Cancel is the only options.

Expected results:
The ability to add buttons or group of buttons.
