Steps to Reproduce:
1.Navigate to Automate->Customization->buttons 
2.Try to add new button or button groups

Actual result:
No forms displayed for adding button/button groups. see attached screenshot.


Expected results:
button/button groups form should be displayed

Adding button/button groups is working successfully. 

Verified in 5.6.0.5-beta2.4.20160503153816_1fb554f
