How reproducible:
100%

Steps to Reproduce:
1.Navigate to Compute->Infrastructure->PXE
2.Go to Customization Templates accordion 
3.From Configuration button Add a New Customization Template
4.Add another Customization Template with the same name

Actual results:


Expected results:
The UI should display the "Name has already been taken" flash error message.

This change validates the name uniqueness within each pxe_image_type.
