Adamw filed a bug against upstream gtk, complaining that the virt-manager toolbar has a big icon with gtk 3.13.3 https://bugzilla.gnome.org/show_bug.cgi?id=732708 vm_clone_wizard is only available in 48x48, in an icon theme directory that is not marked as scalable, but fixed size. The gtk behavior has changed to respect icon theme directory configuration (unless you specify the force-scale flag when loading), but really, virt-manager should install its icons in the size it wants to use them in.
