Another user experienced a similar problem: I reproduced this by following instructions from comment 15. I marked old root on lvm partition to be reused as new root (with reformating) and mark all others existing partitions (boot and swap on the same lvg) to be deleted. And I created new /boot and swap of the same size as on previous installation.
