Description of problem: Version-Release number of selected component (if applicable): 5.5.2.4.20160127105142_395c086 How reproducible: every time Steps to Reproduce: 1. Automate -> Customize -> Service Dialog. 2. Create a dialog with a Drop Down List element configured for dynamic values, "Refresh" button checked and don't initialise the list. 3. Save 4. Order from the Service Catalog using the above service dialog. 5. Repeatedly select different options from the drop down list. Actual results: every time an item is selected from the dynamic drop down list an additional "Refresh" button is added to the UI. Expected results: there should only ever be the one (1) "Refresh" button associated with the drop down list box. Additional info:
