Description of problem:
When editing saved external provider, most of the fields are missing. 
For example, editing VMware provider, the next fields are missing:
vCenter
ESXi
Data Center
Cluster
Verify SSL certificate checkbox
and proxy host

This issue is relevant also for Xen and KVM providers.

Steps to Reproduce:
1. Browse webadmin -> administration -> providers.
2. Select saved provider and click edit.
3.

Actual results:
Most of the fields required are missing except username and password

Expected results:
Provider relevant fields should appear

How isn't not a test blocker and a regression? Do you have a workaround?

Medium severity because the simplest workaround is to delete the provider and add it again and it's not really affecting import functionality because it's still possible to fill this fields in import dialog.
