'Add' button is disabled while adding new catalog for Red Hat Virtualization(rhv41) provider

Steps to Reproduce:
1. Add 'Red Hat Virtualization'(rhv41) provider
2. Navigate to services > catalogs 
3. Add 'new catalog items' for 'Red Hat Virtualization'

Actual results:
Add button is disabled

Expected results:
Add button should be enable to add service catalog data
