Description of problem:

One of our customers opened a case and let us know that upstream bug https://bugs.launchpad.net/octavia/+bug/2024188 affects his RHOSP 16.2 deployment: when adding a member to a pool via the octavia ui in Google Chrome, the text fields for weight and port number don't display correctly. This prevents us from adding members to a pool and from submitting the form.
